Minors and not self-sufficient person
Form to be printed and filled out and given to Y-40® at check-in
UNDER 14 YEARS OLD
If there are minors under the age of 14 unaccompanied by their parents, download the form and have the parents fill it out and sign it. 
MINORS BETWEEN 14 AND 17 YEARS OLD
If there are minors between 14 and 17 years of age unaccompanied by their parents, download the form and have it filled out and signed by the accompanying person who is responsible for them.
