General regulations, terms of sale and user information
PARTICIPANT IN ACTIVITIES – NOTICE: This general regulation of Millepini Immobiliare S.r.l. – Nova S.S.D. a R.L. (hereinafter collectively referred to as “Hotel; Thermal Baths; Spa; Millepini; Y-40®; pool or facility”), valid until 31/12 of the current year, contains contractual obligations for the undersigned and provides necessary information regarding the activities carried out, as required by consumer protection regulations.
You are therefore asked to read it carefully in its entirety and to request any explanations necessary for full understanding before signing.

Forms
Form to be printed, filled out, and submitted to Y-40® at check-in.
Diver medical questionnaire
For activities with a Y-40® instructor:
- First freediving experience
- First scuba experience
- Dive with a guide or safety
Form for minors and non-autonomous individuals
If present:
- Minors under 14 years old: download the form and have it completed and signed by the parents.
- Minors aged 14 to 17: download the form and have it completed and signed by the accompanying person responsible for them.
- Non-autonomous individuals: if under 18, follow the instructions above; if adults, download the form and have it completed and signed by the accompanying person responsible for them.
Authorization for accompaniment / pickup of minors
If someone is to be authorized to accompany or pick up a minor or a non-autonomous individual:
- A separate form is required for each minor or non-autonomous individual.
- A form must be completed for each occasion that authorization is needed.
